At Northeast Cincinnati Pediatric Associates, we have established a number of policies that are intended to make our interaction with your family more predictible and improve the efficiency with which we can provide treatment. For your convenience, we have also posted a number of commonly used forms that you may be asked to complete when you visit one of our offices. These forms are available in PDF format, and are available for you to download, print, and complete before coming to the office. By completing forms ahead of time, you can minimize the amount of time you spend at our office.

Online Appointment Requests and More

Northeast Cincinnati Pediatric Associates now offers online appointment requests. You can now complete a simple online form to make non-urgent appointment requests. Once your request is submitted, our staff will then process the request and contact you for confirmation within one business day.

This service is meant only for non-urgent requests, and messages sent via this service are only retrieved during normal business hours. This means if you need to schedule a same-day sick visit, you should still call whichever of our offices is most convenient for you.

This service is NOT intended as a medical answer line, and clinical questions will not be answered through this service. Please make an appointment to discus clinical questions with your child's pediatrician or nurse practitioner.

As with any piece of communication that comes through our office, schedule requests that come to us via this form are subject to the same privacy policies. However, information sent via this form may not be considered secure. If you have concerns about the privacy of any piece of information required on this page, please call our office instead.
